looking at the form, i see that the customer can order from a pre-defined list of products. is it possible to add a product that also requires a text field to also be completed by the customer.
such as "Demo product 1" - "text field" - "price" - "Qty"

Sure - Would they be typing in the price and qty?
No, the price would be already set, they just need to input the unique ref no and the quantity.
In fact it would be even better if the "Demo Product 1" would be a drop down box, where they could select either "Demo 2" or "Demo 3" and the price change accordingly.
You could use a drop down for the item.
