I send 2 email 48hrs ago without any responses yet. I need to be up and running in 12hrs :S

I need to know how I can customize the form into my site...

Example..
This is what I did, but as soon I click on “Continue” am not longer in my site, the form will be by himself… I would like to have the preview, submit and error page stay into my site..

PLEASE HELP!

You need to customize the template within the /interface/ folder to match your site design.

Which template? Can you please clarify... Are you talking about "legend_form.php" ? This file?

Yes,

You can see which template your form is using by clicking on the [edit] link on the
form list via the admin.

Scroll down to Part 3 in middle of the site..

I replied to your ticket. You want to go about it this way:

(1) Customize the form template to have your site design.

(2) Once (1) is completed, when you view the URL to the form it would have the site
design.

(3) You can then view the source of the form (first page) and paste that into your
existing page.

You do not want to paste the actual form template file / code into your existing page.
FYI

I just reply to your email, please ticket #MHP-131196
